SMS Consent Proof

1. OVERVIEW

USLY LLC provides booking software for appointment-based businesses. Some businesses using USLY may offer optional transactional SMS updates during the booking flow.

These messages may include appointment confirmations, cancellations, reminders, and booking-related support messages.

2. HOW CONSENT IS COLLECTED

End users provide their mobile phone number during a USLY-powered booking flow.

The SMS consent checkbox is shown inside the booking widget before booking is finalized.
The checkbox is optional.
The checkbox is not pre-selected.
Booking can still continue even if the user does not consent to SMS.
Transactional SMS messages are sent only if the user explicitly provides consent.

3. CONSENT TEXT SHOWN TO USERS

By providing your phone number, you agree to receive transactional SMS related to your appointment, including confirmations, cancellations, reminders, and booking-related support messages. Message frequency varies. Message and data rates may apply. Reply STOP to opt out. Reply HELP for help.

This consent is stored as part of the platform’s consent and audit system and is associated with the user’s booking flow.

5. CONSENT AUDIT TRACKING

Accepted SMS consent is stored and tracked in the USLY admin interface together with the consent title, version, acceptance timestamp, scope, and source.
